Forms Related to the IB at Signature

The table below contains a link to forms that you will need as an IB full-diploma candidate at Signature. All of the forms are in PDF and have the typewriter tool enabled so that you can complete them, print them, sign them, and either turn then in by hand or scan them to submit electronically. In most cases, you will submit the form to the principal, the counselor, or to the IB coordinator.


	Form Title and Link	Purpose
1.	Alternate Credit Permission Form	Use this form to seek permission to earn course credit at an institution other than Signature School. Students who take courses outside of Signature risk not earning credit for these courses if they do not gain prior approval through this form. Note that course credit achieved at an institution other than Signature School will not earn a weighted grade.
2.	Request for Exemption from Automatic Re-Enrollment	Use this form to seek permission to exempt yourself from or to defer automatic re-enrollment in a course in which you have earned a D or below. This form should only be used by seniors who will not graduate without exemption or by IB full-diploma candidates who cannot go "full IB" without deferment.
3.	General Regulations Form	All IB full-diploma candidates have to submit this form testifying that they have read the General Regulation of the IB Diploma Programme as stated by they IBO.
4.	Exit Form	We hope you never need this form. However, occasionally students find that they need to exit the full diploma program. These students, after meeting with the involved parties (parents, teachers, and administrators), sign this form to indicate their understanding of the ramifications for exiting the program.
5.	Drop/Add Form	Use this form to drop or add a class. The form must be received by counselor's office by the end of the school day on the second Friday of the new semester.
6.	Study Hall Request Form	Use this form to request a study hall.
